MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Program Assistant – Adult Day

Pay Rate: $16 per hour

Responsibilities:

Provide daily care and support to health-impaired older adults or individuals with brain injuries, intellectual/developmental disabilities, emotional or behavioral disabilities, or mental health diagnoses.
Assist with planning and facilitating individual and group activities.
Ensure the care, safety, and engagement of individuals through recreational and social programs.
Support individuals with activities of daily living (ADLs), including hygiene, bathroom assistance, grooming, meal preparation, and medication administration.
Provide guidance in building social skills, self-advocacy, and self-esteem.
Support independence and participation in daily program activities.

Qualifications:

High school diploma or equivalent; must be at least 18 years old.
Minimum of one year of experience working with individuals with intellectual and developmental disabilities, preferably older adults.
Physical exam within the past 12 months and current TB test. We will provide if you do not have this.
Proficient in monitoring vital signs, supporting ADLs, and promoting independence.
Certified in CPR and First Aid.
Strong communication skills, attention to detail, and a compassionate, dependable attitude.
