Job Specifics
- Join a growing team of providers in leading the way for embedding technology into primary care.
- Deliver high-quality, safe patient care through a collaborative team model instead of individual patient panel structure.
- Strong clinical abilities for management of a population age ~55+ with multiple chronic health conditions
- Patient care settings will include clinic appointments, virtual visits, group visits, taking care to their home and hybrid options.
- Supports creating a culture of innovation growth, encourages open discussion of ethical and clinical concerns and willingness to give and receive real time feedback.

- Clinical base hourly pay range for this position: $60.39$73.68.
- Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.
REQUIRED QUALIFICATIONS: Education: Master's degree in advanced practice nursing . Experience: Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients serve. Licensure/ Certification: IL Advanced Prac Nurse IL Reg Nurse Fed Controlled Sub Lic BLS/ACLS Certified Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: Education: N/A Experience: Experience with EPIC electronic health record. 1-2 years of experience as an Advanced Practice Nurse Licensure/ Certification: N/A Other Skills/ Knowledge: N/A
