Your Role as a Case Manager

• Collaborate with an interdisciplinary team to create patient-centered treatment plans.
• Facilitate discussions on discharge plans and contingency strategies.
• Monitor the patient experience and maintain high-quality care standards.
• Lead weekly team meetings to adjust treatment plans as needed.
• Complete necessary documentation and maintain compliance with regulations.
• Conduct patient and family education on rehabilitation and the role of the Case Manager.
• Assist in the coordination of resources to anticipate and prevent discharge delays.
• Participate in utilization reviews and case management on-call duties as required.

What We Look For in Our Candidates

• Qualifications:
• Must be licensed to complete assessments independently in your discipline (e.g., RN, SW, OT, PT, ST).
• Active state licensure is required where applicable.
• Must meet eligibility for either CCM® or ACM certification; certification must be obtained within two years of starting the position.
• Education & Experience:
• Nursing candidates must have at least an Associate Degree in Nursing (BSN preferred).
• Other health professionals must hold a Bachelor’s degree (Master’s degree preferred).
• Preferred 2 years of rehabilitation experience.
