We're seeking a dedicated and compassionate Social Worker to join our team! In this rewarding role, you'll provide evaluation, counseling, and support to clients within our Adult Day Health Programhelping them achieve greater independence and improved quality of life in their home and community.

Location: Columbia / Owings Mills, MD

Schedule: MondayFriday | Day Shift | Full-Time (40 Hours)

Salary: $70,000/year + Sign-On Bonus of $1,500

What You'll Do:

• Conduct comprehensive evaluations, gathering data through records, interviews, and collaboration with team members.
• Develop and implement individualized treatment plans in coordination with physicians and interdisciplinary teams.
• Continuously assess progress, updating goals and modifying plans as needed.
• Provide one-on-one counseling to clients and/or family members in the home or community setting.
• Prepare accurate and timely documentation, including evaluations, progress reports, and discharge summaries.
• Participate in team meetings to review client status and support goal setting.
• Submit billing and documentation promptly.
• Communicate effectively with case managers and team members.
• Perform related duties as assigned to support client care and program success.

Qualifications:

• Master's Degree in Social Work (MSW) from an accredited program.
• Active Maryland Social Work License required.
• Reliable transportation to travel between centers.
• Strong communication, empathy, and organizational skills.
