Job Description

American Traveler is seeking an experienced RN Case Manager with inpatient experience for a day shift contract, requiring an active MD or Compact RN license.

Responsibilities

• Work in an inpatient hospital setting on the Case Management unit
• Manage an average daily census of 15 cases
• Responsibilities include providing and coordinating medical home care, placement, and social services for discharge planning
• Day shift, 5x8 hour days with 8:00 or 8:30am start times
• Holiday coverage required as needed
• Travelers are first to float within the hospital as needed and must be comfortable floating within their scope
• Coordinate and manage patient care for safe and effective discharges including wrap-around services
• First-time travelers are accepted for this position
• Mandatory hospital orientation modules must be completed prior to starting
• Return travelers must have been away for at least 6 months; former staff must be gone for 12 months before returning as travelers
• Most locations allow one contract extension before a six-month break from the facility is required
• RTO requests must be included at the time of initial consideration and cannot exceed 7 days; if more than 4 consecutive days, additional week may be added to contract
• Block schedule or schedule restrictions will not be accommodated

Requirements

• Active MD or Compact RN license required at time of consideration
• Minimum of 2 years of inpatient case management experience
• Preferred experience with Epic EMR
• Two professional references from most recent employers required; at least one must be a supervisor or manager and the other may be a charge RN
• CPI certification may be required depending on the unit
• American Heart Association BLS required; ACLS and PALS if unit-specific
• NRP certification required if applicable for the unit
• Permanent residence must be at least 50 miles from the facility
