Become the Case Manager You Aspired to Be

  • Collaborate with an interdisciplinary team to shape treatment plans tailored to patient needs and preferences.
  • Coordinate with team members to develop tentative discharge plans and contingencies.
  • Participate in planning and executing the patient discharge experience effectively.
  • Monitor patient experiences related to quality, timeliness, and care appropriateness.
  • Facilitate weekly team conferences and coordinate treatment plan adjustments.
  • Complete necessary case management documentation and addendums.
  • Stay updated on regulations, company policies, and operational standards.
  • Analyze case management reports and initiate appropriate actions based on Key Care Indicators.
  • Attend meetings to identify trends and collaboratively mitigate acute care transfers.
  • Engage with patients and families within 24 hours of admission.
  • Assess patient goals and complete necessary documentation within 48 hours of admission.
  • Educate patients and families regarding rehabilitation processes and the role of the Case Manager.
  • Schedule and facilitate family conferences as needed.
  • Assist patients in timely procurement of resources to prevent discharge delays.
  • Oversee compliance with regulations concerning orthotics and prosthetics.
  • Make timely referrals and ensure all documentation for post-discharge providers is accurate.
  • Participate in utilization review processes and trend analyses.
  • Join the on-call schedule as necessary for case management duties.

Qualifications for Your Success

  • Required License or Certification:
    • Must be qualified to complete assessments independently based on your discipline (e.g., RN, SW, OT, PT).
    • If licensure is necessary for your discipline within your state, it must be active.
    • Must meet eligibility for CCM® or ACM certification upon entry or within two years of joining.
    • CCM® or ACM certification is required to be obtained within two years for the Case Manager II position.
  • Minimum Qualifications:
    • For Nursing roles, an Associate Degree in Nursing is required, with RN licensure; a BSN is preferred.
    • For other eligible licensed or certified healthcare professionals, a minimum of a bachelor’s degree is required; graduate degrees are preferred.
    • A minimum of two years of rehabilitation experience is preferred.
