Hospice Nursing Case Manager

Key Responsibilities for a Hospice RN / RN Case Manager:

• Serve as the primary contact for patients and their families, helping them navigate each stage of their hospice journey.

• Provide skilled, hands-on nursing care according to each patient's individualized Plan of Care, delivered in their home or assisted living facility.

• Conduct initial and ongoing assessments to monitor patient conditions and adapt care as necessary.

• Collaborate with a multidisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to develop personalized care plans.

• Offer emotional, physical, and spiritual support to both patients and their loved ones, providing education and bereavement guidance.

• Educate caregivers on safely and confidently caring for their loved one, combining practical training with emotional support.

• Communicate important updates regarding patient conditions to the care team for prompt intervention.

• Maintain accurate clinical documentation and adhere to infection control and compliance standards.

• Contribute to the organization's quality improvement and scheduling initiatives to ensure consistent, compassionate, and efficient care.

• Assist patients and families in understanding key aspects such as: medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes.

About You: Qualifications - What You'll Bring:

• Active RN license in your state of employment (or eligibility to obtain one).
• 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar nursing fields.
• A genuine passion for hospice care: compassionate, empathetic, and patient-centered.
• Comfortable delivering care in various settings, including private homes and health facilities.
• Strong communication and critical thinking skills in sensitive emotional situations.
• Valid driver's license, auto insurance, and reliable transportation are required.
• Current CPR certification.

Preferred Experience (Not Required):

• Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
• Experience as Admissions RN, in wound care, float pool, or in travel nursing.
• Experience working with terminally ill patients or serious illness support.
