Scope of Work: Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Qualifications:
- Bachelor's degree or equivalent required.
- Master's degree or equivalent preferred.
- 2 years of relevant experience required.

- Or CRT-Registered Dietitian (RD) CDR Commission on Dietetic Registration Upon Hire Required
- Or LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan Upon Hire Required
