Title: Physical Therapist – Home Health
Location: Colchester, VT
Duration: 20 weeks (starting 12/01/2025)
Shift: Day, 5x8-Hour (08:00 – 16:30), weekend rotations required

Job Summary:
The Home Health Physical Therapist provides skilled therapy services to adult patients in a home health setting. Responsibilities include assessing, planning, and implementing individualized care plans, conducting patient/family education, and completing documentation in EMRs. Therapists will see 5–6 patients per day and must be flexible with holiday coverage. Reliable personal or rental transportation is required.

Required Skills & Experience:

Active state license as a Physical Therapist
BLS certification
Minimum 3 years of clinical experience, including home health (required)
Prior travel experience required (at least 2 previous assignments preferred)
Strong assessment and documentation skills
Ability to work with various EMR systems

Preferred Skills & Experience:

Experience with a wide range of orthopedic and neurologic conditions including hip/knee replacements, spinal surgery rehab, Parkinson’s disease, MS, TBI, and stroke recovery
OASIS documentation experience
Knowledge of assistive devices, prosthetics, and home exercise program planning

Additional Notes:

No block scheduling allowed
Must cover weekend rotations and holidays as needed
Public transportation not accessible—vehicle with all-season or snow tires required Nov–Apr
