Job Description

American Traveler is seeking a Physical Therapist with at least 1 year of home health experience for a full-time day shift assignment.

Responsibilities

• Home health setting providing care to patients in their residences
• Service areas include White River Junction, Lebanon, Bradford, and Windsor across Vermont and New Hampshire
• Day shift schedule
• Average visit length is approximately 45 minutes
• Responsibilities include evaluating patients, developing and implementing treatment plans, and documenting goals and progress
• Educate patients and their families on treatment rationale and procedures
• Coordinate care and communicate effectively with the interdisciplinary healthcare team
• Role offers autonomy with strong team support and a focus on meaningful patient relationships
• No radius restrictions for local candidates

Requirements

• Bachelor’s degree in Physical Therapy required
• Current licensure as a Physical Therapist in VT and NH or an unencumbered compact license required if applicable
• Minimum 1 year of home health PT experience required
• Current BLS/BCLS certification required
• Rehabilitation certifications such as ASHA, CHT, or ACP Training are required if applicable
