Job Description

American Traveler seeks an experienced RN Case Manager with at least 5 years of case management experience and a WA or compact RN license for a hospital-based role.

Responsibilities

• Position is based in a hospital case management unit
• Care for a diverse patient population including NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU patients
• Caseload ranges from 15 to 30 patients depending on staffing
• Works four 10-hour day shifts per week (0700-1730) with every other weekend rotation
• Floating required throughout the hospital
• EMR used is Epic; experience preferred
• No on-call responsibilities
• Holidays required per department rotation
• Orientation includes NEO and two weeks in the department
• Main responsibilities include clinical assessment, case management, and discharge planning for patients across multiple units
• Role involves discharging patients to SNF, IP, LTAC, shelter, ALF, and for Medicare/Medicaid patients, including homeless populations
• Partner with case management social workers, assistants, and a manager for daily support
• Scrubs in any solid color or business casual attire are acceptable
• Free parking is available on site
• Up to seven days of RTO may be approved

Requirements

• Active WA or compact state RN license required
• Minimum of 5 years case management experience
• BSN and BLS required
• Experience with diverse patient populations including NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU
• Must have strong case management skills including discharge planning and working with complex patient needs
• Epic EMR experience preferred
• First-time travelers will not be considered
