Job Title: Hospice Case Manager • Registered Nurse • Days
Join our team as a Hospice Case Manager, a role requiring a compassionate and dedicated Registered Nurse. This 13•week contract position involves a 40•hour work week, Monday to Friday, during day shifts.
Responsibilities:
• Oversee hospice care for assigned patients
• Adapt to different territories as required
Requirements:
• Valid MA Nursing License
• BLS Certification
• At least 2 years of Hospice/Home Health Nurse experience
• Proficiency in Hospice case management
• Familiarity with OASIS; Homecare Homebase (HCHB) experience is a plus
Additional Information:
• No on•call or specific scrub color requirements
• Contract extension opportunities available
• PPE details to be determined
• Pay cycle: Sunday to Saturday
• COVID•19 vaccination not mandatory
