Skills: Adult protective services, Biopsychosocial assessment, Child protective services, Complete intake and admission screenings, Coordinates with internal and external team members, Develop plan of care to meet patient/caregiver needs, Discharge Planning, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Documents pertinent information to facilitate care coordination, Educates patient/caregiver to facilitate discharge, Establish and revise goals, Facilitate discharge and contingency planning, Family therapy, Group Therapy, High risk intervention patient vulnerability or safety risk, Individual counseling, Knowledge of cultural issues, Participate in development of treatment plan, Perform Bio-psychosocial assessments, Provide patient and family education, support and advocacy, Psychiatric, Psychiatric Disorders, Psychiatric Hospital, Suicide/homicide/self-harm, Understanding and utilization of short-term therapy, Work with multi-disciplinary team to ensure continuity of care
