Schedule: Part-Time

Shift :8:00am-12:00pm Mon-Friday. Can be Flexible.

Location: Office location is 150 Medical Memorial Pkwy, Palm Coast. Our Inpatient Units are located at -AH Fish 1055 Saxon Blvd, Orange City, FL 32763 and AH Palm Coast HHA and Hospice 150 Memorial Medical Pkwy, Palm Coast, FL 32164

The role you'll contribute:

Responsible for delivering various social work services to terminally ill patients and families. This position provides initial psychosocial evaluation, ongoing psychosocial support, direct casework services, and referral to community resources to assist with end-of-life care needs. Additionally, this role collaborates with the hospice interdisciplinary team to implement, evaluate, and adjust individualized plans of care. In addition, this position assists with Medicaid applications, Nursing Home and ALF placements, hospice inpatient unit placements, and financial needs. Additionally, this role engages in community education and outreach. This position aims to support patients and families in reaching desired outcomes through effective social work interventions.

• Provides counseling to patients and their families.

• Understands and addresses end-of-life issues.

• Works independently while maintaining case load expectations.

• Collaborates with interdisciplinary teams to implement and evaluate care plans.

The expertise and experience you'll need to succeed:

Minimum qualifications:

• Master's degree

• 1 year experience in health care setting

• Active American Heart Association BLS

• Valid in state Drivers License with current car insurance

Preferred qualifications:

• EMR experience

• Hospice experience

• Hospice Medicare Conditions of Participation

• Clinical Social Worker License (LCSW)
