Grow into the Case Manager You Aspire to Be

• Collaborate with interdisciplinary teams to assess and guide treatment plans tailored to patient needs.
• Coordinate discharge plans and ensure contingency plans are ready.
• Monitor and facilitate the patient discharge experience for seamless transitions.
• Participate in weekly team conferences to modify treatment plans as needed.
• Maintain accurate documentation and case management addendums.
• Stay informed of regulations and policies relevant to your practice.
• Engage in Acute Care Transfer meetings to identify trends and develop solutions.
• Meet with patients and families within 24 hours of admission to establish effective communication.
• Conduct assessments and case management documentation within the required timeframe.
• Educate patients and their families about rehabilitation and the role of the Case Manager.
• Assist in timely resource planning to prevent discharge delays.
• Ensure compliance with regulations regarding orthotics and prosthetics ordering.
• Facilitate referrals and maintain accurate discharge information.
• Participate in the utilization review process for continual improvement.
• Engage in the case management on-call schedule as necessary.

Essential Qualifications

• Active license to independently complete assessments based on your discipline (RN, SW, OT, PT, ST, Rehabilitation Counseling).
• For nursing roles, a minimum of an Associate’s Degree in Nursing is required; a BSN is preferred.
• For other eligible professionals, a minimum of a Bachelor’s Degree is required; a graduate degree is preferred.
• Two years of rehabilitation experience is preferred.
