Social Worker
JOB SUMMARY: The Social Worker (MSW) provides essential social work and care management services to participants in the PACE program, delivering comprehensive support to improve their quality of life and independence in the community. Key responsibilities include conducting participant screenings, managing cases, providing counseling, and making referrals, along with organizing and implementing social work services customized for participants and their families.
The Social Worker serves as a crucial link between the interdisciplinary team, caregivers, and community agencies, ensuring that participant needs are thoroughly addressed and services are seamlessly coordinated. This position plays a vital role in advocating for participant well-being and supporting the objectives of the PACE program.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conduct initial assessments, care planning, and periodic reassessments (at least every six months) for participant cases, ensuring timely completion adhering to departmental standards.
- Accurately document participant changes and updates in the medical record, maintaining compliance with industry standards.
- Effectively communicate and collaborate with Interdisciplinary Team (IDT) members regarding participant status, needs, and care plans.
- Keep case management records updated, ensuring comprehensive documentation is maintained.
- Coordinate 24-hour care delivery and provide individual and family counseling to support participants and caregivers.
- Organize and lead group counseling sessions and activities to meet participant needs.
- Refer participants and families to suitable community resources, advocating for their access to necessary services.
- Foster positive relationships among team members, promoting teamwork and cohesion.
- Consult with staff on the social, emotional, and cultural factors affecting participant health and advise on available community social services.
- Engage in program and policy development for social work services and contribute to Quality Improvement (QI) initiatives.
- Conduct home visits to participants' residences to assess and support their individual needs.
- Actively participate in IDT meetings to contribute to care planning and team discussions.
- Ensure compliance with the Social Workers' Licensing Act of 1991 and relevant state regulations while providing services.
- Perform additional duties and special projects as required.
EXPERIENCE AND EDUCATION:
- Master's degree in Social Work from an accredited school recognized by the Council on Social Work Education.
- Membership in the Academy of Certified Social Workers (ACSW) or another NASW-recognized certification is preferred.
- Current professional license in the relevant state.
- A minimum of two (2) years of experience working on an interdisciplinary team in a healthcare setting such as a hospital, nursing home, or within the community is highly desirable.
- At least one year of experience working with a frail or elderly population is preferred; training will be provided if needed.
PRE-EMPLOYMENT REQUIREMENTS:
- Reliable transportation, a valid driver's license, and minimum required liability auto insurance.
- Medical clearance for communicable diseases and all immunizations must be up to date prior to engaging in participant contact.
- Successful completion of a comprehensive criminal background check, which may include federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference checks, and a drug screening.
