Summary
Job Title: Social Worker Care Management
Location: Carroll Hospital, Westminster, MD
Overview: Carroll Hospital, part of the LifeBridge Health system, is seeking a compassionate and skilled Social Worker Care Management to join our dedicated care team. This full-time position plays a critical role in supporting patients and families through the continuum of care, ensuring psychosocial needs are assessed and met in collaboration with interdisciplinary teams.
Key Responsibilities:
- Gather and assess comprehensive information to identify patient and family needs.
- Develop a psychosocial database, perform assessments, and create intervention plans based on findings.
- Collaborate with interdisciplinary and inter-agency teams to address post-acute care needs.
- Integrate social, cultural, emotional, and spiritual factors into patient care and discharge planning.
- Provide crisis intervention and counseling to assist patients and families in adjusting to medical conditions.
- Maintain accurate documentation of assessments, interventions, and outcomes per hospital policy.
- Partner with case managers, healthcare providers, and third-party payors to ensure timely and efficient service utilization.
- Coordinate and participate in discharge planning rounds; communicate regularly with payor representatives and the treatment team.
- Educate teams on care progression, appropriate bed utilization, and denial prevention strategies.
Qualifications:
Education: Master's degree in Social Work (MSW) required.
Licensure: Active LMSW, LCSW, or LCSW-C license required in the state of Maryland.
