Master of Science Degree in Social Work and LCSW certification required

Responsibilities

As a Social Worker, you will provide direct patient care within a patient's home. You will have the satisfaction of knowing you are ensuring the patient receives optimal care, by assessing, planning, implementing, re-evaluating, and revising care and coordinating resources. Among other responsibilities, you will update and maintain patient records and implement orders for medications and treatments.

• Completes the initial psycho-social assessment and develops a plan of care for social services
• Assesses the social and emotional factors in order to estimate the patient's capacity and potential to cope with problems of daily living
• Provides social work intervention through counseling and education for patients to maximize coping capacity
• Identifies and coordinates community resources for the patient
• Instructs the patient and family to understand, accept, and comply with medical recommendations
• Provides services that promote and restore the patient to optimum social and emotional adjustment within capacity of the patient
• Assists the physician and other disciplines in understanding the social/emotional factors related to the patient's health problems

Qualifications

Master of Science degree in Social Work
Licensed Bachelor Social Worker / LBSW certification
Previous home health, hospice, skilled nursing, rehab facility, hospital, mental health clinic, or child welfare setting experience preferred
Familiarity with medical software and equipment - MatrixCare/Wellsky a plus
Participation in multi-disciplinary patient case conferences, staff meetings, and in-service education programs

Salary Range

USD $35.00 - $38.00 / Hour
