Major Responsibilities:
Demonstrates expertise in relevant area of practice. Provides clinical support to colleagues, solving complex problems and using specialty knowledge to assist in patient care
Supports department operations in compliance with local, state, and federal regulations, accrediting agencies, and with organizational policies and procedures
Participates collegially with members of the department of pharmacy and other health care teammates to ensure pharmacy representation in the development of medication use management programs including clinical guideline development, critical pathways, disease management, drug monographs/class reviews/order sets and drug use programs. Serves as the clinical pharmacy liaison to the site or enterprise service line, applying expertise in drug therapy to prevent and resolve drug-related problems.
Designs, educates, evaluates and leads safety and performance improvement initiatives, including development of educational programs for medical, nursing, and pharmacy teammates to achieve the organization's clinical quality, cost, and service quality goals
Identifies opportunities for stewardship of high cost or high use medications in area of practice to limit waste, generate revenue, and/or reduce overall cost of care. Executes approved action plans and tracks stewardship initiatives.
Mentors, orients, trains, and assists in the performance evaluation of pharmacy teammates, residents, and students as assigned
Supports team leader in executing initiatives to meet the needs/goals of the pharmacy enterprise
Performs duties of relevant pharmacy teams as called upon, which may include order verification, medication preparation, patient counseling, medication management, transitions of care support, and utilization management reviews. Pharmacists involved in the provision of services covered under a collaborative practice agreement may be required to demonstrate competence and achieve credentialing in the areas of chronic disease they are managing.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Licensed in the state(s) of practice or eligible for reciprocity required
Immunization and non-vaccine injectable certification required based on area of practice.
Credentialing and privileging required based on area of practice.
Education Required:
Bachelor's or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP
Experience Required:
Requires completion of pharmacy residency training or equivalent experience or working experience as a licensed pharmacist in area of specialty
Direct experience with training, education or preceptor roles required.
PGY1 residency or 4 years of hospital pharmacy experience preferred

Excellent communication and organization skills
Excellent presentation, facilitation, and training development skills
Demonstrates decision making, team building, customer service, and conflict resolution skills
Life support training courses may be required dependent upon department discretion
Physical Requirements and Working Conditions:
Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 35 pounds of material without assistance
Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity
May be required to frequently stretch, bend, squat, or kneel to perform job
Utilizes appropriate personal protective equipment (PPE) to minimize exposure to hazardous agents
Takes necessary precautions to avoid inadvertent needle sticks, minor cuts or bruises
Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines
Must have use of both hands, be able to see clearly and read small font such as that on unit-dose medication packages and syringes
Must have functional speech, vision, and hearing
Operates all equipment necessary to perform the job
Exposed to normal office environment
May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Protective clothing and equipment must be worn as necessary when warranted
