We are currently seeking a dedicated Mental Health Therapist to be part of our innovative Collaborative Care Model (CoCM) team.

As a key player in our team, you will provide essential clinical services in a primary care environment, working alongside Optum Behavioral Care to support the mental and physical well-being of patients. This exciting role involves collaboration with both patients and healthcare professionals to create positive outcomes and a seamless care journey.

This position is embedded in Woodhaven Pediatrics and follows a Monday to Friday schedule - no evenings, weekends, or holidays are required.

As a hybrid role, you will work remotely for one or two days each week, with the remaining days spent onsite at the medical clinic.

Key Responsibilities:

• Utilize both virtual and in-person methods to coordinate care with patients' medical providers and other necessary mental health professionals.
• Conduct screenings and assessments for a range of mental health and substance use disorders.
• Monitor treatment progress and changes in patients' clinical conditions actively.
• Assist in psychotropic medication management, focusing on adherence monitoring and effectiveness.
• Deliver brief evidence-based interventions tailored to individual patient needs.
• Engage in regular consultations with psychiatric consultants to optimize treatment options.
• Create and implement relapse prevention plans for patients nearing discharge from care.

Required Qualifications:

• Current licensure in Michigan as a:
  • Licensed Master Social Worker (LMSW)
  • Licensed Professional Counselor (LPC)
  • Licensed Marriage and Family Therapist (LMFT)
• Experience in providing behavioral health services to pediatric patients.
• A minimum of 3 years treating behavioral health conditions.
• At least 1 year of experience in integrated primary care settings.
• Background in using evidence-based psychosocial treatments.
• Eligibility to enroll as a provider in the Medicare program.

Preferred Qualifications:

• Experience in the Collaborative Care Model (CoCM).
• Exceptional organizational and follow-up skills.
• Ability to maintain professional relationships with patients and care team members.
• Experience working with patients in both in-person and virtual formats.
• Comfort with technology solutions and adaptability to new tools.

Compensation for this position ranges from $58,800 to $105,000 annually, depending on various factors, including local labor markets and relevant experience.
