Job Summary

The Palliative Care Social Worker is an integral member of the Palliative Care interdisciplinary team, providing psychosocial expertise and support to palliative care patients/families through the palliative care services consult team. The Palliative care social worker applies knowledge of psychosocial care in support of palliative-focused quality improvement initiatives.

Qualifications

Education

Master’s Degree in Social Work

Experience

Previous experience in a healthcare setting

Minimum of 2 years clinical experience in a hospital, hospice or long-term facility

Licensure, Certification, Registration

LCSW

Palliative and Hospice Certification

Knowledge, Skills and Ability Requirements 

Comfort with end of life discussions and goals of care discussions.  Public speaking and group facilitation. Microsoft Office products including MS Word, Excel, PowerPoint, Outlook and Access. EPIC lists and reports.  Ability to be a self-starter as well as a team player.
