Job Overview:

  • Facilitates the overall multi-disciplinary Stroke Operational and Peer Review Teams - assessing, developing, planning, implementing and evaluating the overall stroke program.
  • Clinical Coordination *Identifies stroke patients and rounds to proactively identify and/or resolve physician, patient, family issues. *Facilitates implementation and documentation of the progress and outcomes of plan. *Collaborates with physicians, nurses, Physical Therapists, Occupational Therapists, and other departments to evaluate and problem solve variations in care. *Assures pre and post operative education for patients is provided. *Participates in the process monitoring performance improvement indicators in the stroke patient both at the unit level and within the Indiana Region.
  • Program Management *Assesses and prioritizes needs for protocols, guidelines, pathways, and standard order sets relating to the care of the neuro stroke patients in consultation with Clinical and Administrative Program Managers and the Medical Director. *Coordinates the communication and implementation of standards to the clinical staff through both formal and informal settings. *Maintains and monitors documented standards through direct observation, medical record review, staff feedback, and other appropriate sources. *Collects appropriate data for entry into quality databases. *Gathers and analyzes data used to plan for marketing and budgeting. *Develops and implements a marketing plan in conjunction with the Public Relations and Regional Network Services departments. Monitors effectiveness of plan. *Prepares regular reports monitoring program achievements. *Coordinates monthly Stroke Operational and Peer Review meetings. *Coordinates stroke site visit for re-designation every two years; prepares and submits applications to the Joint Commission timely. *Develops processes and policies to meet and/or exceed TJC standards as specified by accrediting agency TJC.
  • Quality Improvement *Oversees resource utilization and patient care outcomes. *Develops and implements a quality plan which is interdisciplinary and patient-outcome focused. *Initiates plan of action to resolve identified quality issues. *Monitors stroke core measures closely with the assistance of the Quality department and improves processes in data collection to ensure real time data collection and follow up. *Maintains current knowledge of external and internal stroke core measure indicators and standards.
  • Education and Outreach *Develops and implements programs for hospital care givers based on identification of staff learning needs in order to assure that the standard of care for the neuro stroke patient is constantly maintained. *Works closely with Public Relations and Regional Network Services to market the program to Primary Care Providers, community leaders, referring hospitals and the public. *Acts as a liaison to other referring hospitals for educational needs as well as promoting advancing care in their organization to become level III stroke ready facilities.
  • Coordinates "Get With the Guidelines" stroke data base to ensure data entry is accurate and validates any information being reported. Collaborates with the Quality Analyst responsible for stroke measures to ensure QI reporting is meeting needs for regulatory requirements; I.e. The Joint Commission and CMS.

Required: Certifications/Licenses/Experience: Active Registered Nurse (RN) in Indiana or other compact licensure state

Preferred Certification/License/Experience: BSN
