Chronic RN

Our client, a leading healthcare provider, is seeking a Chronic RN to join their team. As a Chronic RN, you will be part of the patient care team supporting individuals with End Stage Kidney Disease (ESKD). The ideal candidate will have strong communication skills, a compassionate approach to patient care, and the ability to work collaboratively, which will align successfully in the organization.

Job Title: Chronic RN

Location: Harrisburg, PA

Shift: 8 hours per shift

What's the Job?

• Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients.
• Perform ongoing collection and analysis of patient data during hemodialysis treatment and document findings in the patient medical record.
• Educate patients and families about treatment options and care plans, ensuring they understand their health needs.
• Administer medications as prescribed and document medical justification and effectiveness.
• Participate in emergency response measures and serve as a resource for the healthcare team.

What's Needed?

• Graduate of an accredited School of Nursing with current state licensure.
• Minimum of 1 year of Nephrology Nursing experience in the last two years.
• Chronic hemodialysis experience is required.
• Current CPR BLS Certification.
• Ability to work collaboratively with a diverse healthcare team.
