As a Mental Health Counselor, you will provide essential clinical services to patients in a Collaborative Care Model (CoCM) within a primary care clinic. You will collaborate closely with the primary care provider and psychiatrists to enhance both the mental and physical well-being of your assigned patient caseload.

Our team operates at the forefront of mental health care, utilizing both virtual and in-person modalities to ensure our patients receive the best possible support. The role is centered in our healthcare client, Summit Medical Group, with a Monday through Friday schedule, allowing for work-life balance with no evenings, weekends, or holidays.

Key Responsibilities:

• Coordinate care through virtual and in-person sessions with patients' medical providers and additional mental health professionals as necessary.
• Screen and assess patients for common mental health and substance abuse disorders.
• Track treatment responses diligently while monitoring any changes in symptoms and side effects from treatments.
• Support medication management as prescribed by medical professionals, ensuring adherence and assessing treatment efficacy.
• Deliver brief, evidence-based interventions utilizing techniques such as motivational interviewing and cognitive behavioral therapy.
• Engage in regular consultations with psychiatric consultants to optimize treatment plans in collaboration with medical providers.
• Assist patients in developing self-management plans for continued success post-discharge.

Required Qualifications:

• Current licensure in Tennessee as either an LCSW, LMSW, LMFT, LPC, or other qualifying mental health credentials.
• A minimum of 3 years of experience treating behavioral health conditions.
• At least 1 year of experience in primary care or a similar integrated medical-behavioral health setting.
• Proficient in evidence-based psychosocial treatments.
• Eligible to enroll as a provider in the Medicare program.

• Experience in the Collaborative Care Model.
• Strong organizational skills with a keen attention to detail.
• Ability to build professional relationships with patients and care team members.
