Schedule:   Monday - Friday 8AM-4:30PM In Office

Allay Hospice is seeking a licensed LPN or RN with strong clinical and communication skills to join our team as a Hospice Intake Coordinator in Brookfield, WI . This is a critical role that helps patients and families transition smoothly into hospice care by coordinating the intake and admission process from start to finish. As a n Intake Coordinator You will:

  G uide patients and families through the hospice intake and admission process with care and clarity .

  n
n
• n

  Review referrals and medical records to ensure patients meet hospice eligibility .

  n
n
• n

  Triage incoming referrals and coordinate urgent or same-day admissions when needed .

  n
n
• n

  Educate patients and families on hospice services, answering questions and easing concerns .

  n
n
• n

  Coordinate services, equipment, and care needs with our team and external providers .

  n
n
• n

  Verify insurance, obtain authorizations, and document all referral and patient information .

  n
n
• n

  Communicate with physicians, discharge planners, and other healthcare professionals to ensure continuity of care .

• n

  Ensure all regulatory and compliance steps are followed throughout the intake process .

  n

• n

  Serve as a knowledgeable resource for hospice criteria and support referral sources with alternatives if needed .

Qualifications

n
• n

  Registered Nurse or Licensed Practical Nurse, with current and unencumbered state licensure.

  n

• n

  At least two years of healthcare experience, preferably in hospice operations.
