About the Role
We’re seeking a compassionate, client-focused Licensed Clinical Social Worker to join our team in a hybrid role, blending in-person and remote work. You’ll provide both therapeutic and administrative support, helping clients achieve improved mental health and overall wellness in an integrated care setting.

Key Responsibilities

Conduct comprehensive client assessments, including interviews and history reviews
Create and implement individualized treatment plans with measurable goals
Collaborate with the care team to provide updates and coordinate services
Deliver crisis assessments and initiate interventions as needed
Facilitate referrals to healthcare providers and community programs
Promote holistic wellness and integrated care approaches
Maintain accurate and timely clinical documentation
Support administrative and care coordination activities

Qualifications

Master’s degree in Social Work (MSW)
Active LCSW license in New York
Enrolled in NY Medicare/Medicaid (required)
1–2 years of clinical mental health experience
Bilingual (English/Spanish) strongly preferred
Comfortable with hybrid work (some in-person attendance required)
