📋 Job Overview

• Evaluate assigned patients and develop, implement, and modify individualized treatment plans based on patient needs and physician direction.
• Delegate and direct therapy services appropriately.
• Focus on patient safety, functional rehabilitation, and return to community activities.
• Collaborate with interdisciplinary teams to overcome discharge barriers.
• Comply with documentation standards, regulations, and facility policies.

🧠 Duties & Competencies

• Conduct comprehensive evaluations and clinical assessments.
• Develop goal-oriented treatment plans aimed at discharge readiness.
• Lead individual, concurrent, and group therapy sessions.
• Provide family and caregiver education and home exercise programs.
• Maintain accurate and timely EMR documentation .
• Participate in student supervision , professional development, and advocacy.
• Follow all regulatory, ethical, and safety standards (10-Foot Circle of Safety principles).

✅ Requirements

• License : Must hold a valid, active State PT license
• Certifications :
  • BLS (Basic Life Support) – Required before first shift
• Experience :
  • Minimum of 2 years of clinical experience required
• Documentation Skills : Must be proficient in daily notes, evaluations, discharge planning, and insurance compliance
• Compliance : Must meet all standards set by state boards, accrediting bodies, and Select Medical
