Position Summary

As a Licensed Practical Nurse, you will play a crucial role in managing and delivering exceptional resident care. Your responsibilities will include:

• Allocating staff duties in line with policies, promoting teamwork for optimal care delivery.
• Conducting rounds to assess residents and reporting any concerns to appropriate personnel promptly.
• Receiving shift reports from charge nurses and preparing a comprehensive report for the Director of Nursing.
• Effectively communicating areas of concern that require administrative attention.
• Managing staff performance and fostering accountability to improve patient care.
• Developing measurable goals for staff and encouraging professionalism in the workplace.
• Documenting and communicating care plans and interventions accurately.
• Conducting resident assessments and actively participating in revising care plans in collaboration with Charge Nurses.
• Ensuring thorough resident and family education to promote health awareness.
• Maintaining a thorough understanding of emergency procedures and safety issues.
• Completing Monthly Quality Assurance Performance Improvement (QAPI) activities and reviewing pharmacy recommendations.
• Taking on additional duties as assigned.

Qualifications

• Current LPN license in the state of practice
• Preferred: Two years of supervisory experience in long-term care
• Three to five years of long-term care or relevant nursing experience
• Excellent verbal communication skills for effective interaction with residents, families, and staff
• Strong time management and organizational skills
• Ability to stand and walk for extended periods
• Capability to lift and transfer residents weighing over 200 lbs using proper techniques
• Proficient in moving equipment weighing over 40 lbs
• Manual dexterity and skillful hand use for resident care
• Able to perform necessary documentation
• Visual acuity for patient assessments
• Ability to manage multiple activities efficiently

Pay range: $61,604 - $97,375 annually
