We are seeking a dedicated Licensed Clinical Social Worker to play a crucial role in enhancing the lives of our patients. In this position, you will collaborate with a comprehensive health care team to coordinate services that promote independence and improve the quality of life for our members. Your expertise will support individuals in navigating their care effectively.

Key Responsibilities:

• Conduct thorough problem identification and psychosocial assessments to evaluate patient needs, social support, and health status.
• Manage and coordinate an assigned caseload effectively, ensuring timely access to appropriate medical services.
• Develop and monitor personalized plans of care in collaboration with nurse case managers and other health care professionals.
• Identify and advocate for community resources to meet patients’ needs, ensuring cost-effective and efficient implementation.
• Maintain meticulous documentation of all case management activities in compliance with organizational standards.
• Analyze patient and program outcomes to enhance the quality and cost-effectiveness of care provided.
• Assist patients in applying for local, state, and federally funded programs and report any suspected cases of abuse or neglect.
• Provide supportive counseling and education to patients and their families, including guidance on end-of-life issues and Advanced Directives.
• Stay updated on current case management standards and innovations in patient care to maintain high-quality social work practices.

Note: This role requires availability every other weekend and on holidays.

Qualifications:

• Minimum of three years of clinical experience, plus two years in case management.
• At least one year of experience with the defined patient population.
• Master’s degree in Social Work (MSW) is required.

Licensure:

• Licensed Clinical Social Worker certification from Virginia and Maryland within six months of hire.
• Independent Clinical Social Worker License from the District of Columbia within six months of hire.
• Case Manager Certificate is required within 36 months of hire.

Preferred Qualifications:

• Proficiency with computer software in a Windows environment.
• Knowledge of community resources and regulatory issues in the Mid-Atlantic region.
• Inpatient social work experience is advantageous.

This position is part-time, and you’ll work a total of 24 hours per week. Your schedule will consist of day shifts, with workdays including Tuesday, Wednesday, and alternating weekends. Join us at Virginia Hospital Center - UR-Discharge Planning, and help us make a difference in our community.
