Key Responsibilities:

1. Provide culturally sensitive, non-judgmental end-of-life care for patients, caregivers, and families.
2. Conduct thorough assessments of psychosocial needs, connecting families with community resources as necessary.
3. Offer social work interventions, including advocacy, counseling, and crisis intervention, tailored to patient and family needs.
4. Document evaluations and interventions, ensuring proper communication within the interdisciplinary team.
5. Engage in continuous education to stay current with best practices in social work and hospice care.
6. Act as a liaison to community resources and participate in public engagements to enhance community visibility.

Qualifications:

• Master's degree in social work from an accredited institution with a minimum of three years of clinical experience preferred.
• Current state license for LCSW, if applicable.
• Strong knowledge of social work ethics and practice standards.
• Ability to work independently and collaboratively in a team environment.
• Commitment to maintaining confidentiality and compliance with HIPAA regulations.
• Proficient in computer software and office equipment.
• Valid state driver's license and proof of auto insurance meeting Connecticut state requirements.
