Qualifications
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
High School Diploma/G.E.D (Required)
Graduate of an accredited Licensed Practical Nursing program (Required)
Two years of recent LPN experience
One year of home care, hospice or home infusion experience preferred
Previous experience in a discharge planning, preferred
Requires knowledge in nursing, evidence-based practice methods, and person-centered care concepts
Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure
Current Cardiopulmonary Resuscitation (CPR) required
Drivers License
LPN IV certification preferred
Current Basic Life Support (BLS) certification from American Heart Association, preferred
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10
lbs. constantly
This is a Home Health LPN Intake role that will work weekdays with some weekends. The hours are 8:00am to 5:00pm. We are looking for at least 2 years of LPN experience.
