Position Overview

As a Healthcare Licensed Practical Nurse, you will play a vital role in ensuring quality resident care. You will assign staff in accordance with established policies and promote teamwork to deliver timely and effective care. Your responsibilities will include:

• Conducting rounds to assess residents and their care.
• Communicating concerns and needs to appropriate staff promptly and professionally.
• Receiving and reporting details from charge nurses, maintaining clear documentation.
• Identifying and addressing any events needing administrative attention.
• Managing, mentoring, and empowering staff members to promote accountability in resident care.
• Overseeing patient care standards and skillfully delegating responsibilities as necessary.
• Developing measurable goals for staff while fostering professionalism in the workplace.
• Serving as a reliable resource for nursing personnel and seeking guidance when needed.
• Documenting and communicating care plans effectively to ensure understanding and compliance.
• Assessing the physical, psychological, and emotional needs of residents.
• Collaborating with Charge Nurses to maintain clinical competencies and intervene when necessary.
• Maintaining a thorough knowledge of emergency procedures and safety practices.
• Completing monthly Quality Assurance Performance Improvement (QAPI) reviews and monitoring pharmacy recommendations.
• Performing additional duties as assigned.

Qualifications

• Current LPN license in the state of employment.
• Preferred: Two years of supervisory experience in long-term care.
• Three to five years of long-term care or similar nursing experience.
• Strong communication skills for effective interaction with patients, visitors, and staff.
• Excellent time management and organizational skills to meet productivity standards.
• Physical ability to stand and walk for extended periods.
• Capability to lift heavy objects, as well as assist in moving residents over 200 lbs using proper techniques.
• Ability to push and pull equipment over 40 lbs effectively.
• Skilled in manual dexterity and coordination for effective care and equipment handling.
• Competent in performing necessary documentation.
• Visual acuity to ensure accuracy in assessments.
• Able to manage a variety of activities in a dynamic healthcare environment.

Compensation: $61,604 - $97,375 annually
