Discharge Planner - Case Management

FT/DAYS located at Corona Regional Medical Center in Corona, CA

Reporting to the Case Management Director/Manager, this position provides evaluation and psychoeducation to patients and their families or significant others, to assist them in coping with psychosocial problems related to acute and chronic illness or injury. Assess patient current living environment and general medical/social needs. Provides intervention on order to maximize the patient's re-integration into the home and community. Able to provide competent care for patients throughout the lifespan. Integrates discharge planning process.

The tradition of caring that culminated in the establishment of Corona Regional Medical Center began in 1965, with the founding of Corona Community Hospital. Since that time the hospital has changed to meet the needs of the rapidly growing community. Corona Community Hospital merged with Circle City Medical Center in 1992 and the resulting entity became Corona Regional Medical Center. The merged hospitals are now a 238-bed community hospital network comprised of a 160-bed acute care hospital and a 78-bed rehabilitation campus. Position Requirements

• Certificate as Social Service Designee or one years experience working in the field as a Social Work Assistant or discharge Planner in lieu of Certificate, or Bachelor's degree in psychology or sociology with equivalent job related
• Minimum of 12 months clinical experience in the field of Mental Health
• Age specific education and/or experience working with adult and geriatric behavioral health
• Excellent written and verbal communication
• Current BLS (CPR) Certificate
• Knowledge of Patient
• Experience with discharge planning and case
