Shift: 8 hr day (08:30–17:00)
Schedule: Initially self-scheduled, then based on department needs; call required; float as needed
Case Load: Up to 25 patients
Scrubs: Business casual provided

Unit Overview

• Acute Hospital Case Manager experience is a HARDSTOP
• INTERQUAL certification/knowledge is a HARDSTOP
• Must be able to organize and participate in family meetings, develop treatment plans, and arrange DME, SNF, and ambulance transport independently
• Weekend rotation required
• Documentation via Epic

Required

• Certifications: BLS
• Experience: Minimum 2 years acute medical care manager experience performing care coordination and utilization review
• Skills:
  • Admission criteria, appeals, audits
  • Concurrent and retrospective review
  • Pre-cert and prior authorization
  • Determine medical necessity per evidence-based guidelines
  • Disease management, discharge planning, plan of care
  • Familiarity with HMOs, IPAs, InterQual, Milliman Guidelines

Preferred

• Previous charge CM experience
• Teaching or community hospital experience
• LTAC experience

Unit Notes

• Travel experience required; first-time travelers can be screened only if highly experienced
• Fast-paced environment requiring independent decision-making
• Patient ratio typically 1:20
