Your Path to Becoming a Visionary Case Management Director
In this role, you will:

1. Oversee the daily operations and human resources management of the Case Management department.
2. Manage the interdisciplinary plan of care and discharge planning process to optimize services, focusing on patient care outcomes and key performance indicators.
3. Advocate for patients and their families, ensuring that their needs are prioritized.
4. Guide and mentor Case Managers, providing training on case management practices, regulations, and organizational objectives.
5. Maintain high-quality standards in operations consistent with organizational goals.
6. Build partnerships with insurance companies, self-insured employers, case management firms, and other healthcare networks.
7. Celebrate the achievements and milestones of our dedicated staff.

Qualifications
To succeed in this role, you should have:

• Current CCM® or ACMTM certification preferred.
• Qualified to independently perform assessments within your discipline’s scope of practice.
• An active license is required if licensure is mandatory for your discipline in the hospital's state.
• A Bachelor's degree in Nursing (BSN) with RN licensure for nursing roles; a minimum of a Bachelor's degree for other eligible healthcare roles, with a graduate degree preferred.
• A minimum of three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• Flexibility to work weekdays, weekends, evenings, and nights as needed, including holidays.
