• Deliver high-quality nursing care in post-acute care facilities
• Monitor residents' conditions and inform the nursing team of any changes
• Check and report on residents' vital signs
• Maintain a clean and safe environment through tasks like sanitizing surfaces and changing linens
• Assist residents with personal hygiene, feeding, grooming, and daily living activities (ADLs)
• Reposition and transport residents within the facility as required
• Facilitate communication among staff, residents, and family members
