We are seeking a highly motivated and experienced Case Manager to join our dynamic team. Our ideal candidate will have a passion for social work, with a focus on dealing with homeless and mentally ill individuals, and a deep understanding of the nonprofit sector. This role requires a minimum of 2 years of experience in a similar position. You will be responsible for coordinating and providing case management services, including creating treatment plans and coordinating care with other professionals. Responsibilities:
1. Develop, implement, and review individual case management plans for clients, including assessment of needs, setting goals, and coordinating services.
2. Coordinate with various stakeholders such as social workers, psychologists, and other healthcare professionals to ensure comprehensive care.
3. Advocate for clients' needs and rights, and assist with crisis intervention as needed.
4. Maintain accurate and up-to-date client records, and ensure compliance with all relevant laws and regulations.
5. Provide emotional support and counseling to clients, and refer them to appropriate resources as needed.
6. Monitor and evaluate clients' progress, and adjust case management plans as necessary.
7. Conduct regular meetings with clients to review their progress and address any issues or concerns.
8. Collaborate with other team members to ensure a comprehensive, integrated approach to client care.
9. Participate in ongoing professional development and training opportunities to stay current with best practices in case management and social work.
Qualifications:
1. Bachelor's degree in Social Work, Psychology, Counseling, or a related field.
2. A minimum of 2 years of experience in case management, preferably in the nonprofit sector.
3. Previous experience working with homeless and mentally ill individuals.
4. Excellent interpersonal and communication skills, with the ability to build strong relationships with clients and other stakeholders.
5. Strong organizational and case management skills, with the ability to manage multiple cases simultaneously.
6. Knowledge of relevant laws and regulations related to case management.
7. Ability to work effectively in a team-oriented environment.
8. Strong problem-solving skills, with the ability to think critically and make informed decisions.
9. Ability to handle sensitive information with discretion and maintain client confidentiality.
10. Proficiency in Microsoft Office Suite and case management software.
11. Valid driver's license and reliable transportation.
