Become the Case Manager You Aspire to Be

• Work collaboratively with the interdisciplinary team to guide treatment plans tailored to patient needs and preferences.
• Establish and coordinate tentative discharge plans with the interdisciplinary team.
• Participate in the planning and execution of patient discharge experiences.
• Monitor patient satisfaction in terms of quality, service appropriateness, and expectations.
• Facilitate weekly team conferences and coordinate modifications to treatment plans.
• Complete case management addendums and required documentation efficiently.
• Stay updated on regulations, company policies, and department operations.
• Analyze case management reports and develop action plans accordingly.
• Understand payor requirements and ensure compliance for orthotics and prosthetics.
• Participate in Acute Care Transfer meetings to identify trends and reduce transfers.
• Conduct initial patient and family meetings within 24 hours of admission.
• Manage patient goals and complete assessments within 48 hours of admission.
• Educate patients and families on rehabilitation processes and the Case Manager's role.
• Schedule family conferences as needed to ensure comprehensive care.
• Assist patients in securing resources promptly to prevent discharge delays.
• Make timely referrals to post-discharge providers and maintain accurate documentation.
• Participate in utilization review processes and contribute to data collection and trend evaluation.
• Be part of the case management on-call schedule as required.

Qualifications

• License or Certification:
• Must be able to independently complete assessments within the scope of practice for their discipline (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).
• If required, must hold an active state license in their discipline.
• Must meet eligibility requirements for CCM® or ACM certification by entry date or within two years in this position.
• CCM® or ACM certification is a requirement, or it must be obtained within two years of being placed in the Case Manager II role.
• Minimum Qualifications:
• For Nursing roles, a minimum of an Associate Degree in Nursing is required; RN licensure with a BSN preferred.
• All other eligible licensed or certified healthcare professionals must hold at least a bachelor's degree, with a graduate degree preferred.
• A minimum of 2 years of rehabilitation experience is preferred.
