Become the Case Manager You Aspire to Be

• Collaborate with an interdisciplinary team to design treatment plans tailored to patient needs.
• Establish discharge plans alongside healthcare professionals to ensure continuity of care.
• Monitor patient experiences and treatment efficacy regularly.
• Lead weekly team meetings to discuss treatment modifications and patient progress.
• Document and maintain comprehensive records in accordance with company policies and regulations.
• Analyze key indicators and reports to strategize improvements in patient care.
• Educate patients and families about rehabilitation and the role of case management.
• Avoid discharge delays by assisting patients in securing necessary resources promptly.

Qualifications

• Must possess a license or certification in your discipline (RN, SW, OT, PT, ST, etc.) and be able to conduct independent assessments.
• Active state licensure is required if applicable for your profession.
• CCM® or ACM certification required or must be obtained within two years of starting.
• A minimum of an Associate Degree in Nursing (BSN preferred) or a Bachelor’s degree in your healthcare discipline is necessary.
• Two years of rehabilitation experience preferred.
