Your Role as a Case Management Specialist

• Work collaboratively with an interdisciplinary team, guiding treatment based on patient needs and preferences.
• Establish tentative and contingency discharge plans with the team.
• Plan and execute patient discharge experiences to ensure smooth transitions.
• Monitor and analyze patient experiences concerning quality, service, and payor expectations.
• Facilitate weekly team conferences to coordinate treatment plan modifications.
• Complete case management documentation and remain knowledgeable of regulatory standards.
• Review and analyze case management reports to plan actions effectively.
• Understand commercial contract levels and payer requirements for appropriate patient care.
• Participate in acute care transfer meetings to identify trends and improve processes.
• Meet with patients/families as required to enhance communication and care.
• Support patients with timely resource procurement to avoid discharge delays.
• Ensure compliance in ordering orthotics and prosthetics.
• Make timely referrals to post-discharge providers and document appropriately.
• Participate in the utilization review process to improve care delivery.

Qualifications

• License or Certification: Must hold an active license to independently complete assessments within your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• If licensure is required for your discipline, you must maintain an active license.
• Must meet eligibility for CCM® or ACM certification upon entering the position or within two years.
• CCM® or ACM certification is required or must be obtained within two years of assuming this role.

• Minimum Qualifications:
  • For Nurses: Minimum Associate Degree in Nursing and active RN licensure (BSN preferred).
  • For all other licensed health care professionals: Minimum bachelor's degree (graduate degree preferred).
  • 2 years of rehabilitation experience preferred.
