Our team at MGB is expanding and hiring a Bilingual (Spanish and English) Community Health Worker II to support our patients and community!
This role requires frequent travel within the community, including ER clinic-based work, in-home patient visits and accompanying patients to physician appointments.
The position requires the ability to work flexible hours including possible occasional evenings or weekends based on patient needs.
Job Summary
The Opportunity
The Complex Care Community Health Worker (CHW) is a trusted, frontline worker who is a respected member of and/or has a close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/ link/ intermediary between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
The CHW will work in an interdisciplinary team that includes Registered Nurses, Social Workers, and Non-Clinical Coordinators. They will assist patients in navigating complex medical, behavioral, and social systems of care to promote wellness and self-sufficiency. CHWs play a critical role in improving health outcomes, addressing health-related social needs, and reducing barriers to care.
Qualifications
What You'll Do
- Build and maintain trusted relationships with patients, families, and communities to improve care outcomes.
- Conduct outreach and engagement activities, including home visits and community-based follow-ups.
- Serve as a bridge between patients and clinical/non-clinical services, helping navigate primary care, behavioral health, and specialty care systems.
- Assist patients in addressing social determinants of health such as housing, food insecurity, transportation, employment, and health insurance access.
- Collaborate with care teams to develop person-centered care plans based on patient goals and values.
- Perform structured assessments and develop SMART goals with patients to guide care coordination.
- Help patients set and achieve short-term and long-term health goals.
- Provide culturally and linguistically appropriate education and support.
- Reinforce care team instructions and provide health coaching, health promotion, and disease prevention support.
- Provide follow-up, monitor progress, and encourage adherence to care plans and appointments.
- When appropriate, facilitate communication and understanding between patients and healthcare providers.
- Advocate for patients and their needs both within and outside the healthcare system.
- Support patients in completing applications for public programs including SNAP, MassHealth, and housing assistance.
- Document all patient interactions in the electronic medical record (EPIC) accurately, promptly and in accordance with NCQA documentation requirements and payor contract expectations.
- Refer patients to appropriate internal and external services and escalate concerns to licensed clinicians as needed.
- Participate in regular case review meetings, supervision, and team huddles.
- Actively contribute to a culture of teamwork, respect, and shared decision-making.
- Demonstrate reliability, accountability, and adaptability in fast-paced healthcare settings.
- Uphold ethical standards of practice, including confidentiality and informed consent.
- Adhere to organizational policies related to safety, infection control, and appearance.
- Maintain up-to-date knowledge through continuing education, certification, and professional development opportunities.
- Demonstrated commitment to patient advocacy.
- Bachelor's Degree required; experience in lieu of degree may be considered.
- Bilingual in Spanish and English highly preferred (Speak, Read and Write).
- Experience or strong familiarity with the community being served is required.
- Knowledge of community resources is required.
- 1+ years' experience in community outreach, care coordination, case management, or health navigation required.
- Experience working with patients and families dealing with complex medical and psycho-social issues with an ability to help patients understand and access resources required to support care goals required.
- Ability to work effectively in a complex fast paced medical environment and multiple practice locations.
- Experience working in collaboration with interdisciplinary healthcare teams preferred.
- Prior experience as a community health worker, peer specialist, peer advocate, or in a similar role preferred.
- Experience working with a variety of community agencies and resources preferred.
- CHW certification by the Massachusetts Board of Certification (or eligible to become certified within 12 months of hire). Support available after 6 months of successful employment.
- Demonstrated understanding of and commitment to health equity and social justice.
- Exceptional interpersonal, communication, and motivational interviewing skills. Excellent oral and written communication skills.
- Strong critical thinking and problem-solving skills
- Ability to apply trauma-informed, culturally responsive, and patient-centered care approaches.
- Exceptional organizational and time management skills, with attention to detail. Demonstrated ability to set priorities and follow through with tasks.
- Proficient with technology including electronic medical records, mobile apps, and virtual care platforms.
- Ability to work independently and collaboratively in various environments (clinic, home, community).
- Familiarity with local community resources and social service systems.
- Commitment to ongoing professional development and participation in supervision.
- Solid knowledge of the Core Competencies for CHWs (as identified by Massachusetts, Department of Public Health)
Working Model and Hours
- Monday through Friday ET hours.
- Hybrid working model includes minimum of 3-4 days on-site and/or traveling in the field.
