Salary: $120k - $140k DOE
Schedule: M-F / Limited On call as needed
Qualifications
- Bachelor’s degree
- 4+ years previous hospice, home health or case management experience, or equivalent
- Demonstrated ability to effectively direct personnel and utilize mentoring and motivational skills
Certifications and Licenses
- Licensed to practice as a registered nurse in the state of operation
- Must be a licensed driver who can travel to all locations
Responsibilities
- Develop, coach, and serve as a resource and liaison to the Admissions Team and all staff involved from referral inquiry to admission
- Ensure compliance with federal, state, and local regulatory requirements during the referral and admission process
- Manage patient and family care, including assessment, planning, implementation, and evaluation
- Coordinate with physicians, hospice staff, and other agencies to ensure optimal care
- Evaluate staff productivity, referral, and admission data regularly to support staffing and meet budgetary goals
- Monitor referral and admission reports to coordinate follow-up with leadership
- Mentor staff on referral follow-up activities to meet conversion rate goals
- Participate in leadership meetings and strategic planning
