Scope of Position :
The Wraparound Services Case Manager must possess the skills and desire to coordinate available resources ensuring that clients have every opportunity to change their lives. They will provide direct support services to clients seeking assistance from Salvation Army as well as receiving referrals from community partners. They must be able to network with other agencies and advocate for the clients' needs all in an effort to successfully transition into stability.
Knowledge, Skills and Abilities Required :
  1. Must demonstrate a passion for and be familiar with The Salvation Army's mission; must represent the ideals of The Salvation Army in every aspect of his/her work.
  2. Must be able to manage a variety of projects at any given time.
  3. Must be able to work in an environment that is busy and must be able to work in spite of interruptions and surrounding activities.
  4. The ability to maintain a self-motivated and self-directed schedule. The employee must demonstrate the ability to plan, organize, and coordinate the functions of his/her position.
  5. The ability to present oneself in a professional and appropriate manner along with the ability to maintain confidentiality regarding information and documentation.
  6. The ability to interact with coworkers and the public in a positive manner and the ability to work with people of diverse backgrounds and circumstances.
  7. The employee works with sensitive and confidential material. This position requires a criminal history information check. Conviction of a crime will not automatically preclude appointment. The circumstances involved in the conviction will be considered.
  8. Preferable: valid driver's license and a good driving record ((Note: Prior to appointment, finalists will be required to provide authorization to The Salvation Army to obtain a driving record.))
  9. The following equipment may be used: telephone, computer equipment, fax/copy machine, and 10 -key calculator. Frequent phone and in-person contact with people from diverse backgrounds will occur.
  10. Must have the ability to: sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis, to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead, and to lift up to 25 lbs.
  11. Must regularly exercise discretion and judgment with his/her personal contacts and approach to each individual or family.
  12. Maintain mutual respect for clients to ensure trust in relationship-building
Essential Duties and Responsibilities :
It is the responsibility of each employee of The Salvation Army, Medford Citadel Corps, to reach out to people, without discrimination, in the name of The Salvation Army, helping to build whole people- spirit, soul and body- in the name of The Salvation Army.
  1. Conducting follow-up calls or in-person meetings with previous HOPE House participants, including both successful and terminated cases.
  2. Conducting follow-up calls or in-person meetings with current and previous Social Services clients to gather detailed information about service gaps and additional resources they may need.
  3. Assess clients' ongoing needs
  4. Coordinate with Salvation Army Outreach team for referring clients to additional resources.
  5. Work with Outreach team to develop strategies to address gaps in services and resources for our clients.
  6. Create a standard operations procedures guide for the Medford Corps, utilizing TSA Army National guide as reference.
  7. Regular and reliable attendance
  8. Provide clients advocacy and act as a liaison between clients and the public and private service providers in the Rogue Valley
  9. Establish appropriate educational training to enhance the lives of the clients
  10. Aid clients with appropriating adequate transportation to appointments and meetings, when possible, i.e. bus passes, ride-share
  11. Develop groups and materials to educate clients with respect to finances, recovery, general housekeeping, and parenting skills, etc.
  12. Monitor, track and report on the status and progress of all clients
  13. Maintain client files in an orderly manner updating case notes, records, and reports on a weekly basis
  14. Maintain computer records as prescribed by The Salvation Army policy and directed by the Business Operations Director
  15. Maintain hard copy files as a back up to computer records
  16. Complete exit information on clients at time of discharge
  17. Answer the telephone, direct calls and take messages in compliance with The Salvation Army policy
  18. Be familiar with emergency policies and procedures and able to make the necessary contacts with the Fire Dept., Police Dept. and other emergency service agencies as needed
  19. Attend all staff, finance and grant specific meetings as assigned. Attend all supervision meetings with the Business Operations Director.
  20. Attend and complete all grant required and TSA required trainings as they arise
  21. Other duties as assigned
Physical Requirements :
The employee is regularly required to sit for long periods of time operating a keyboard. Ability to stand and/or be physically active for up to two hours or more at a time. Must be able to walk, run, reach and grasp with hands and arms, lift bend, and stoop. The employee may occasionally lift or move up to 25 pounds or more.
Working Conditions:
Works in an office environment with frequent interruptions, multiple deadlines, and competing priorities. Frequent phone and in-person contact with people from diverse backgrounds. Work environment includes managing projects with conflicting requirements and tight deadlines.
MISCELLANEOUS:
The dress code is business causal. As this position works both in an office setting and with frequent community engagement, the employee should dress appropriately for their scheduled activities that day. The Salvation Army holds a high standard for professionalism, and the dress code for employees is no exception.

Education and Work Experience :
  1. Education: High School Diploma or Equivalent
  2. Post-Secondary Education: Preferred Bachelors Degree or equivalent in Social Work or Human Services, with practical application experience
  3. Work/Professional Experience: In lieu of degree, at minimum, four years' experience in Human Services working with a diverse population of individuals and families in crisis who may be under stress.
