Description: This is for PRIMARY CARE and requires experience in both primary care and urgent care settings.Dates: Listed in CommentsPLEASE MAKE SURE TO LIST AVAILABILITY IN BID PRESENTATIONS1. Please specify level of proficiency with EPIC, Provider will be required to use Epic2. Provider must be able to see and treat all patients as the sole provider in the Center3. Provider should have good soft skills as they will be one on one with support staff and each patient4. HCO relies on Agency's credentialing of each provider, HCO will process Medicare Enrollment only5.
