T CASE MANAGEMENT AND DISCHARGE PLANNING EXPERIENCE Training on unit will be minimal - candidates will need to be able to pick up quickly and work efficiently. **REQUIRED: Minimum 5 years of Acute Inpatient Case Management experience (this does NOT include telephonic, utilization management, or therapy experience), Strong computer skills, Epic EMR experience, Nursing degree or MSW, BLS certification through AHA,

current valid MI RN licensure or LMSW with current valid MI licensure, Acute discharge planning experienceDays 8a-4:30p, rotating weekends.MI LMSW License Required
