Social worker must be eligible for LICSW or LGSW -they must have taken and passed the exam already*

Social Worker/Discharge Planner 5x8
Shifts: 800-1630 (8A-430P) 5 x 8 Days

Weekend Requirement: weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed
Floating Requirement: no floating requirement

2 years of experience:Skills Required: Acute HospitalSkills preferred: Long Term Acute Care/Rehab/Skilled Nursing1st TIME TRAVELERS ACCEPTED: as long as they have inpatient discharge planning experience, preferably in acute rehab rehabilitation and using Medconnect
