Job Description

American Traveler is seeking an experienced RN Case Manager for a primary care clinic in Alaska, requiring an active AK RN license and recent case management experience.

Responsibilities

• Position is based in a primary care clinic setting
• Work schedule is Monday through Friday, daytime hours
• Involvement in outpatient and specialty clinic referrals and coordination
• Handles OB case management, including postpartum and newborn follow-up
• Occasional support for video teleconferencing and Exercise Treadmill Tests (ETT)
• May assist with procedures such as colposcopy and cystoscopy
• Responsible for care coordination and case management of high-risk and complex patients
• Collaborates with providers and multi-disciplinary teams to establish individualized healthcare goals
• Facilitates patient referrals and tracks outcomes, serving as a liaison with outside facilities
• Participates in case management reviews and leads team efforts in the clinic
• Active role in patient education, outreach, and documentation
• Maintains infection control, safety, and annual competencies
• May be involved in triage and emergency response within the clinic
• Must respond promptly to patient needs and assist with problem-solving in care coordination
• Periodic on-call rotation and requirements are not specified for this role
• Current or previous local candidates accepted, as no radius rule is stated

Requirements

• Active, unrestricted Alaska RN license required at the time of consideration
• Graduate of an accredited school of nursing; bachelor’s degree preferred
• At least 1 year of case management experience within the last 2 years
• Basic Life Support (BLS) certification required
• National Case Management certification preferred
• Proof of flu vaccination required
• Fingerprint cards required before starting
• Ability to work independently in a fast-paced clinical environment
