Job Description

American Traveler is seeking an experienced RN Case Manager with a current CA license and Epic experience for a day shift hospital position requiring weekend and holiday coverage.

Responsibilities

• Hospital-based case management unit
• Day shift schedule: 5x8 hours, 8:00 am – 4:30 pm
• Required to work two weekends every six weeks
• Floating expected between all facilities within the state
• Must be willing to work at least 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Epic EMR is used and prior experience is required
• Responsible for coordinating and managing patient care transitions and discharge planning
• Completion of mandatory modules pre-start and annually is required
• All RTO requests must be discussed at time of consideration; excessive RTO may delay or affect offer
• Travelers refusing to float may be subject to termination of assignment

Requirements

• Active and unencumbered CA RN license
• Minimum of 2 years' recent case management experience
• At least 2 supervisory references from the last 3 years in a comparable specialty are required for consideration
• Epic EMR experience required
• Cannot have worked as permanent staff at the facility within the past 3 months
• Not eligible if residing within 50 miles of the facility as a returning employee
• Required documentation for consideration includes SS Card and Driver’s License
