Unit Description – Case Manager

Client is seeking experienced RN Case Managers for their Acute Care Case Management team. This is a fast-paced unit with a caseload of up to 25 patients, focused on care coordination, discharge planning, and resource utilization.

Shift Info:

• Shift: 8-hour Day (08:00–16:30)
• Schedule: 4-week schedule, weekend rotation required
• Call: Required
• Float: Required
• Orientation: 40 hours
• Scrubs: Provided (Turquoise)

Systems & Equipment:

• EMR: EPIC (HARDSTOP – required)
• Communication: SBAR

Key Responsibilities:

• Care coordination, discharge planning, arranging DME/SNF/transport independently
• Conduct family meetings and participate in treatment planning
• Knowledge of HMO, IPA, adoption/foster care, substance abuse treatment, homelessness resources, and protective services processes
• No utilization review responsibilities

Required Experience & Skills:

• 2+ years of acute care case management experience required (care coordination focus)
• Experience with interdisciplinary care teams and resource planning
• Must have prior travel experience (HARDSTOP – first-time travelers not accepted)

Certifications:

• BLS required
