π Assignment Overview: RN Case Manager β Discharge Planning
Department: Case Management β General Discharge Planning
Shift: MβF, 8:00 AM β 4:30 PM (5x8s, non-negotiable) Radius Rule: Local candidates accepted.Β
Travelers must reside >50 miles from facility.
β Required Qualifications
- RN Case Manager with minimum 2 years of acute care management and discharge planning experience.
- At least 6 months at a level 1 trauma center or large academic medical center .
- Experience covering multiple inpatient units .
- EPIC charting experience β REQUIRED
- BLS
- RN license (ADN accepted; BSN preferred)
- CCM preferred but not required.
π₯ Facility & Role Details
- Setting: Inpatient Case Management/Discharge Planning
- Caseload: 15β20 patients/day
- EMR: Currently EPIC and naviHealth ; transitioning to EPIC Care Management
- InterQual/Milliman not required, but must have general discharge readiness knowledge.
- Collaboration with:
- Respiratory Therapy and Pharmacy teams for discharge planning (as needed)
- Social Work team (15 FT, 17 PT SWs)
- Reports to: Social Work Manager
- Oversight: Chief of Care Coordination
- Current Team: 15 Case Managers; Goal = 21 (18 general, 3 complex)

- Flu Vaccine β REQUIRED (Only medical exemptions allowed; documentation required)
- Fingerprinting:
- Remote process: Allow 10 days
- In-person: Jackson or Grenada campus (week before start)
π Orientation
- 2-week department-specific onboarding (includes facility orientation)
- Will work alongside current CM team (never alone during orientation period)
π Dress Code
- Any color scrubs or business professional attire
π Schedule Details
- No weekend requirement
- Holiday coverage rotated as needed
